•  Confirm pregnancy

•  Refer to bpas:

1. Complete referral form / letter
2. Complete & sign HSA1 form (GP/clinic dr.)
3. Book appointment on bpas Actionline - 08457 30 40 30
4. If under 19 (or if patient chooses) fax / post completed forms to BPAS consultation centre (01273 508026) or give patient forms by hand to take to appointment
5. If Chlamydia screening offered request results also be forwarded to Wistons
6. Assess and provide ongoing contraception
7. Offer follow up appointment

Post-termination

•  At follow up assess and provide appropriate ongoing contraception (if not previously arranged)
